I was one of the first from the Sheriff's Department to schedule the HRA (Health
Risk Assessment) Blood Draw. Thank God I did! My PSA was high which indicated the
possibility of cancer cells in the prostate. My CareHere Physician was adamant about
me making sure to do a follow up with my regular Urologist. I followed up with another
blood draw and thought I was OK since the levels were quite normal for my age. However,
when I went back to Care Here a few weeks later, they suggested another PSA check
was needed. And they were right. I went to another specialist, numerous tests were
run, and cancer cells were found in 1 of the 10 biopsies that were taken. I thank
God that this problem was found, identified, and treated early! I am expecting great

Rutherford County, Tennessee (CareHere client since December, 2004)
Prior to CareHere implementation, Rutherford County was experiencing an upward trend
in total healthcare costs in excess of 15% year over year. The County Health Plan
had to borrow more than $2 Million to pay claims in excess of reserves.
Four CareHere clinics were installed in December of 2004 to service approximately
3,500 employees plus their dependents across the county. Less than 18 months later,
in July, 2006, the Insurance Director announced the annual trend had been reduced
to 1.7% over the previous 13 months and the reserve fund balance was $13.5 Million.
Sumner County, Tennessee (CareHere client since July, 2004)
Sumner County was experiencing a double digit upward trend in total healthcare costs
prior to CareHere implementation. As of May, 2006, the Insurance Committee reports
annual trend is now at 3%.
Overall office visits processed by Sumner County's TPA are down. Overall costs per
office visit processed by their TPA are down. Primary care patient visit volume
is moving into the CareHere clinic as well as visits by more expensive, chronic
patients.

Two of the most expensive facilities out of 65 implemented CareHere clinics. These two facilities are now among the bottom five in total cost.
Wilson County Schools, Tennessee (CareHere client since September, 2005)
Wilson County was experiencing double digit upward trend in overall health costs prior to CareHere implementation. As of May, 2006, report from an independent actuary shows a -0.5% trend.
