Thanks for your interest in taking the first step towards a "Healthier You" – the
CareHere HRA or Health Risk Assessment. We will give you all the information you
need to take that important step below.
All employees and spouses who are on the Rutherford County Health Plan are eligible
for one Health Risk Assessment every twelve months.
It's the CareHere HRA or Health Risk Assessment, which will ask you a few medical
questions and then someone will check your blood pressure and get a blood sample.
The sample is sent to a lab to analyze your blood for dozens of problems that you
might have without even knowing it. Then in a couple of weeks you will receive a
full color report that will tell how you how you stack up and what you can do about
any problems we find. In fact, if you have a significant problem, we'll even call
you to talk with you about it.
The special HRA Drive is being conducted through May of 2008, when schools and departments
within the county government are striving for recognition in the media for outstanding
HRA participation by the mayor and other elected officials. Note: You can still
schedule your HRA at a CareHere clinic after May, but it would be too late to help
your worksite win recognition.
CareHere is making the HRA as convenient as possible by bringing the HRA to your
worksite or another one nearby. They will be conducted at most schools and several
other county facilities.

Here are three outstanding reasons for completing the HRA:
- You and your spouse are entered into drawings for cash prizes of $500 each month.
At the end of the year, there are drawings of $10,000 for one employee and $4,000
for one spouse.
- You can help your school or department win recognition for outstanding participation
in the HRA Drive. Here are the award levels according to the HRA participation rate:
- Platinum: 90-100%
- Gold: 80-90%
- Silver: 70-80%
- Bronze: 60-70%
- By completing the HRA, you can save yourself and the taxpayers money by catching
problems early – before they became dangerous and costly. There are "silent killers"
- like high levels of cholesterol, blood pressure or blood sugar - that could kill
or disable you. Hundreds of thousands of Americans die every year from heart attacks
and strokes with no previous symptoms. Since CareHere began the Rutherford County
program, hundreds of previously undiagnosed cases of high blood pressure, high cholesterol
and diabetes have been discovered.
Here is the process for setting up your HRA:
- Go to www.carehere.com and
click on "Member Login" to log in. (Note: If you have never used CareHere before,
there are also instructions on the login page to help you register for the first
time, using the Rutherford County access code: RC486).
- Click on "Appointments" on the left side and look for "Health Risk Assessments
Only" to see when we will have a HRA event at or near your work. Please
note that we may not have all the HRA events shown on the schedule yet, but they
will be added as each week as plans develop. There should also be a poster at your
worksite that shows the date we plan to be there, once it is set.
- Click on the date you selected on the calendar at the top of the web page and select
a time that's convenient to you. Then click on that time in the scheduler to set
your appointment. Please mark your personal calendar so you’ll remember it.
- Make sure to fast 12 hours before your HRA. Fasting means no food or beverages except
water or decaffeinated tea or coffee with no sweetener. Wear a shirt or top with
loose sleeves so that it’s easy to roll it up to check your blood pressure and get
a blood sample. Note: You may also want to pack some juice and a snack to have just
after your blood draw.
This valuable HRA is being provided to you at no charge by the
Rutherford County Government - out of concern for you and your family's health.
CareHere follows Federal HIPAA and privacy guidelines. All personal health information
(PHI) is kept private and confidential and provided to parties outside CareHere
only under the following conditions:
- Patient authorized requests (i.e. PHI sent to specialists, hospitals, etc.)
- Request from the designated HIPAA Privacy Officer of patient's employer.
