Lee Anglea
Vice President, Purchasing
Mr. Anglea has worked in the healthcare purchasing arena since 1989. He has hospital, pharmacy and healthcare commodities experience. He manages the procurement of all supplies for CareHere clinics nationwide as well as contract negotiations and vendor selections.
Michelle Anglea, RNC, COHN, CMC
Senior Vice President & Chief Clinical Officer
Mrs. Anglea has a decade of experience in managing onsite healthcare operations.
Most recently, she worked with Fortune 50 Companies overseeing medical sites in
several states.
Mrs. Anglea holds a Bachelor of Science degree in Nursing, with a minor in Psychology,
from Belmont University. She is a Certified Professional coder by the American Academy
of Professional Coders and holds a certification specialty in ambulatory women's
health (NCC).
Mrs. Anglea has deep experience with HIPAA, OSHA, CDC guidelines and NRC regulations
and holds memberships in: American Association of Occupational Health Nurses, American
Health Information Management Association Member, Associate Member of MGMA, American
Academy of Professional Coders, CMSA (Case Management Society of America) Member,
Tennessee Association of Occupational Health Nurses, Red Cross CPR instructor.
Ben Baker, MBA
Chief Operating Officer
Mr. Baker brings 18 years of experience in customer service, information technology,
quality management, and business process improvement from General Electric and IBM
to CareHere.
Mr. Baker is the former President of Precision Data Management Systems providing
Information Technology consulting services. He is a certified Master Blackbelt in
Six Sigma Quality pioneered by GE.
Mr. Baker is a graduate of David Lipscomb University and received his MBA from the
Owen Graduate School of Management of Vanderbilt University.
Mr. Baker and Ernie Clevenger co-founded CareHere in January 2004.
Jackie Bradshaw, LPN
Health Coach
Mrs. Bradshaw obtained a Licensed Practical Nursing certification at Nashville Memorial Hospital in 1991. She worked in the care of asthma and allergy for over 17 years and specializes in educating these types of patients. She has performed all aspects of clinical tasks and has worked with both adults and pediatrics. Mrs. Bradshaw strives to emphasize educating patients on preventive medicine, the importance of compliance with plan of care, and working with chronic care patients to minimize their risk of complications.
Leigh Anne Baker, CLS
Director of Clinic Operations
Mrs. Baker oversees daily clinic operations for CareHere clinics nationwide. She
brings healthcare experience from primary care, pediatric, specialist, and hospital
healthcare environments. Leigh Anne has worked with St. Thomas and Baptist hospitals
in Nashville, TN, as well as in a variety of private practice healthcare settings.
Mrs. Baker holds a B.S. from David Lipscomb University. She is a certified Clinical
Laboratory Scientist by the National Credentialing Agency for Laboratory Personnel
and the Tennessee Medical Laboratory Board.
John Boyd
Director of Software Development
Mr. Boyd has been a software developer for over 8 years and his experience has spanned
over several industries including manufacturing, footwear, and healthcare. His software
development has included web, client-server, and database applications, as well
as database administration. Several of his past projects have been conversions of
legacy applications to newer technologies, such as AS/400 applications to ASP.NET
applications.
Mr. Boyd graduated from Tennessee Technological University in 1998 with a Bachelors
of Science in Business Administration-Management Information Systems.
Kelly Brown
Administrative Coordinator
Ms. Brown brings over 15 years of experience in managing healthcare administrative operations to her role, the last five of which were spent working directly with on-site healthcare. In her current position she oversees benefits, payroll, and general human resources policies.
Ernie Clevenger, MBA, FLMI
President
Since 1983, Mr. Clevenger has worked in the self-funded community in various capacities including:
- Chairman and President of the Self-Insurance Institute of America.
- Chairman of the Workforce for Electronic Data Interface (WEDI) advising United States Department of Health & Human Services.
- President, American Progressive Benefits and Executive Vice President of Alliance Underwriters, two of the nation's leading providers of stop loss insurance for self-insured employers.
Mr. Clevenger is a graduate of David Lipscomb University and received his MBA from
the Owen Graduate School of Management of Vanderbilt University. He received his
FLMI from the Life Management Institute.
Mr. Clevenger and Ben Baker co-founded CareHere in January 2004.
Joc Collignon, MBA
Director of Sales
Mr. Collignon has a background in sales and operations in a variety of industries
including marine, manufacturing, and healthcare. He was recently an employee of
an electronic medical record (EMR) vendor. There, he designed internal and external
marketing materials for the sales force in addition to building pipelines and developing
a mapping tool to help sales representatives with territory management.
Mr. Collignon is a graduate of the University of Tennessee - Knoxville with a Bachelor
of Science degree in Industrial and Information Engineering. He received his MBA
from the Owen Graduate School of Management at Vanderbilt University with a specialization
in Healthcare.
Christie Cornell, MPH
Health Coach
Ms. Cornell is an Exercise Physiologist and Health Coach for CareHere. She has expertise
in areas such as weight management, rehabilitation, and corrective exercise training.
Prior to joining CareHere Ms. Cornell worked as a fitness director for a Corporate
Wellness Center in Virginia and as a group exercise instructor at George Mason University
and Gold’s Gym in Virginia. She also served several years in the medical field of
the U.S. Army.
Ms. Cornell holds a Masters in Public Health from American Public University, and
her BS in Health and Exercise Science from Syracuse University.
Anthony V. Dallas, Jr., MD
Medical Director
Dr. Dallas also practices as a CareHere physician providing onsite care for clients
and overseeing nurse practitioners.
Dr. Dallas attended the University of Kentucky Medical School (Graduated with Honors)
and performed his residency at St. Mary’s Medical Center in Evansville, Indiana.
He is the founder of the Family Healthcare of Hendersonville (Hendersonville, TN)
and is Board Certified by the American Board of Family Practice.
Dr. Dallas holds memberships in: the American Academy of Family Practice (1975),
the American Academy of Medicine (1980), the Sumner County Medical Society (1986),
and the Tennessee Medical Association (1986).
Liza Derbalian, RD, LD
Health Coach
Ms. Derbalian is a Registered Dietitian and Health Coach for CareHere. She has expertise
in areas such as weight management, home care and long-term care nutrition, diabetes
and cardiac nutrition counseling.
Prior to joining CareHere Ms. Derbalian worked as a clinical dietitian for Florida
Hospital in Orlando and as a consultant dietitian at a long-term care facility in
Orlando. She has also held several other positions in the past including- bariatric
nutrition and wt management counseling at a major teaching hospital in NJ, and outpatient
nutrition management at a fitness center in California.
Ms. Derbalian holds a BS in Nutrition from Rutgers University in NJ, and she completed
her dietetic internship at Montclair State University in NJ. Ms. Derbalian is a
Licensed Dietitian in the state of Florida.
Elizabeth Di Biase, RD
Health Coach
Ms. Di Biase is a registered dietitian and health coach for CareHere patients. She
has expertise in many areas of weight management counseling, trauma and burns medical
nutrition therapy, life cycle nutrition and management. Prior to joining CareHere,
Ms Di Biase worked overseas managing several food supplemental nutrition clinics
for the US Military. She has written and presented several articles on celiac disease
and a gluten free diet.
Ms. Di Biase holds both a B.S. in Nutrition and a B.S. in Psychology from the University
of Tennessee at Chattanooga. She completed her dietetic internship in Tucson, Arizona
at University Medical Center. Ms. Di Biase is a certified Weight Management Counselor
with the American Dietetic Association, and a certified Personal Trainer. She is
a member of the Chattanooga District Dietetic Association, the American Dietetic
Association and the Nutrition Entrepreneurs Dietetic Practice Group.
Zach Evans, MBA
Chief Information Officer
Mr. Evans brings over 8 years of experience managing large-scale e-business and
information technology projects to CareHere. In his role he manages CareHere's technology
operations while also participating in the company's strategic planning, sales and
marketing initiatives.
He is a frequent guest lecturer at area colleges and universities on topics such
as marketing, e-business, and online strategies and is an Adjunct Professor in the
College of Business at Lipscomb University. Mr. Evans is a graduate of Lipscomb
University and received his MBA degree with honors from Middle Tennessee State University.
JeNelle Gouvas
Executive Director of Sales and Physician Relations
Mrs. Gouvas comes to CareHere from a major pharmaceutical company where she served
as Executive Sales Director. She was instrumental in the development of training
protocols for new sales representatives, sales, marketing and physician relations.
Mrs. Gouvas will serve in a variety of roles with CareHere, with emphasis on sales,
marketing, physician relations and account management.
Born and raised in Owensboro, Kentucky, Mrs. Gouvas graduated from Western Kentucky
University with a degree in Corporate and Organizational Communication in 1994.
Denise Halbert, RD, LDN
Health Coach
Ms. Halbert is a Registered Dietitian and Health Coach for CareHere patients. She
has expertise in areas such as weight management, cardiac rehab, bariatric nutrition,
and diabetes nutrition counseling. Prior to working for Carehere, Ms. Halbert worked
as the outpatient dietitian at Middle Tennessee Medical Center.
Ms. Halbert holds a Bachelor of Science degree in Family and Consumer Sciences with
a dietetics concentration from Lipscomb University. She also completed the dietetic
internship program from Lipscomb University. She is a licensed dietitian/nutritionist
in both Tennessee and Ohio. She is a member of American Dietetic Association.
Sarah Houser, CPA
Chief Financial Officer
Mrs. Houser is responsible for all financial reporting for CareHere. Additionally,
she handles client billing and payroll. Mrs. Houser joined CareHere in 2004 after
providing accounting services to area start-up companies for five years. Prior to
that she was a senior auditor with Ernst & Young, where she worked primarily with
healthcare companies.
Mrs. Houser is a Certified Public Accountant in the state of Tennessee. She holds
a Bachelors of Science in Business Administration from Auburn University.
Megan Jakes
Operations Coordinator
Ms. Jakes graduated from the University of West Florida in May 2007 with a Bachelors of Arts and Science specializing in Public Relations and a minor in Hospitality, Recreation and Resort Management. Upon graduation, she moved back to her hometown area of Nashville and entered into the corporate world as a Newspaper Liaison with Webster & Associates, an entertainment Public Relations firm. She then moved to HMG Nashville, a radio promotion company. Starting out, Ms. Jakes handled radio promotions for all artists, as well as press releases on specific artists. She was then promoted to Artist Representative in the Sales department and dealt directly with artists and created a development plan for success. Ms. Jakes made the move to CareHere in February 2009 and brings with her organizational, public relations, crisis management and marketing skills.
Suzanne Kalla, RD, LDN
Health Coach
Ms. Kalla is a Registered Dietitian and Health Coach for CareHere. She has expertise
in weight management counseling, renal nutrition, and dietary modification for a
variety of medical diagnoses. Her prior work experience included working with hospital
inpatients and patients on dialysis.
Ms. Kalla holds a B.S. in Nutrition from the University of Tennessee at Chattanooga.
She completed her dietetic internship in Birmingham, Alabama at University Hospital.
Ms. Kalla is a certified weight management counselor with the American Dietetic
Association. She is a licensed dietitian / nutritionist in Tennessee and Georgia
and a member of the American Dietetic Association and the Sports, Cardiovascular,
and Wellness (SCAN) dietetic practice group.
Brendie Keane, RN
Vice President of Health and Wellness
Ms. Keane oversees daily operation of the disease and case management for the patients
of CareHere. She also develops relationships with local health and wellness facilities
to support telephonic disease management of CareHere patients in their own community.
Her most recent work was leading a multi-site health initiative with the YMCA of
Middle Tennessee and Saint Thomas Health Services. She has over a decade of experience
in managing Vanderbilt Allergy and Asthma Research Center where she lead clinical
research initiatives and developed disease management programs for a variety of
respiratory disorders.
Ms. Keane holds a B.S. in Nursing from East Tennessee State University.
Phil Lander
Technology Support Coordinator
Mr. Lander brings over 15 years experience in Information Technology to CareHere.
His experience has spanned several industries including legal, manufacturing, and
the last 9 years in healthcare. He has supported telecommunication systems, client-server
networks, end user systems, as well as managing support teams. In his current role,
Mr. Lander supports the daily infrastructure needs of CareHere, as well as participating
in strategic planning for its growth.
Mr. Lander graduated from the University of Montevallo with a Bachelor’s degree
in Speech-Language Pathology, as well as a Master’s in Audiology.
Tammie Leftwich
Health Coach
Ms. Leftwich is a health coach for CareHere and specializes in working with patients
through the stages of readiness to help them improve their health through substantial
behavioral health changes. She has worked for 20 years as a manager in the healthcare
field. Most recently, Tammie worked with the YMCA of Middle Tennessee and St Thomas
Health Services. In this role, she assessed patient’s individual preventive health
care needs and guided them to the appropriate service. As part of the CareHere team
she will work with patients telephonically in smoking cessation, provide community
resources, and support the patient to meet preventive health guidelines specific
to their needs.
Tammie is a certified facilitator for smoking cessation through the American Lung
Association.
Kathy Mead, MS
Program Coordinator
Ms.Mead has her masters in exercise science and serves as CareHere Program Coordinator.
In this role she is responsible for developing and implementing health programs
such as health risk assessment events, health fairs, speaking engagements, fitness
after an injury, smoking cessation, and weight management for our clients around
the county. She will also provide consults for patients in the area or exercise
and fitness.
Kathy is originally from Fayetteville, GA, a suburb of Atlanta. She attended Vanderbilt
University where she enjoyed an exciting internship at The Late Show with David
Letterman and later graduated in 1998 with a degree in Psychology and Human and
Organizational Development. She received her Master’s Degree from MTSU in 2004 and
began working at The Wellness Center at Nashville’s Baptist Hospital. While at Baptist,
she coordinated the Pre-Natal fitness program, Bariatric surgery prepatory program,
Weight Management program, and Group Exercise program.
Laurie Moore

Debbie Nelson, RN, CCM, COHN
Director of Client Operations
Ms. Nelson brings 30 years of nursing experience with 18 years of hospital, homecare and
occupational health management to CareHere. Most recently, she owned and managed
a workers’ compensation case management business. Ms. Nelson has held positions
as Administrator of a large homecare agency, Director of Patient Services for a
90 bed rural hospital system as well as multiple nursing management positions in
hospital systems in West Tennessee.
Ms. Nelson is a graduate of Union University’s School of Nursing in Jackson, Tennessee,
and is certified as a Case Manager and as an Occupational Health Nurse.
Lauren Oliver, RD, LDN
Health Coach
Miss Oliver is a registered dietitian and health coach for CareHere patients. She
has expertise in the areas of weight management, diabetes nutrition counseling,
and dietary modification for a variety of medical diagnoses. Her prior work experience
included developing an outpatient diabetes education program for Williamson County
Health Department.
Ms. Oliver holds a B.S. in Nutrition with a minor in Business Administration from
the University of Tennessee at Knoxville. She completed her dietetic internship
at Lipscomb University. Upon completion of this program Lauren fulfilled the requirements
necessary to become a Registered Dietitian in the State of Tennessee. She is a member
of both the Nashville District Dietetic Association as well as the American Dietetic
Association.
Jason Pack
Purchasing Coordinator
Mr. Pack brings deep experience purchasing for occupational, primary care, pharmacy,
and corporate health facilities. Most recently, he worked for a organization that
managed hundreds of clinics in 35 different states. For CareHere, Mr. Pack's responsibilities
include: processing supply orders, product research, and order tracking for all
of the CareHere clinics. Furthermore, he oversees formulary pricing for the Director
of Pharmacy Operations.
Mr. Pack holds a Bachelors of Science degree in Exercise Science and Wellness from
Jacksonville State University.
Brandy Ritchey
Director of Client Operations
Mrs. Ritchey brings more than 14 years of experience to her role as Operations Coordinator
to CareHere. She has significant experience in finance, information technology and
operations with both private and public organizations in the healthcare, engineering,
and retail industries.
Mrs. Ritchey graduated from Strayer University with a Bachelors of Business Administration
degree.
Tonya Sanders, LPN

Brooks Smith, PharmD
Director of Pharmacy Services
Dr. Smith brings 12 years of pharmacy experience to his role as pharmacy operations manager. Recently he has been a director of pharmacy services for a hospital pharmacy and has previously been a manager of a retail pharmacy. Dr. Smith will assist on pharmacy issues including formulary management and providing drug information to providers, patients, and clients. Dr. Smith was a cum laude graduate of Mercer University Southern School of Pharmacy in Atlanta, Georgia and is a member of several professional organizations.
Tracy Thompson, RN
Health Coach
Mrs. Thompson is a Registered Nurse and Health Coach for CareHere patients. She received her BSN from the University of South Carolina in Spartanburg, South Carolina. She has extensive experience as a Maternal/Newborn nurse. Prior to joining CareHere she worked at Centennial and Vanderbilt Hospitals which included teaching Labor and Delivery/ Newborn Care classes to new parents.
Ray Tomlinson
Vice President, Florida Operations
Mr. Tomlinson provides executive oversight and direction for CareHere’s Florida
operations from the initial presentation stage through implementation. He works
with CareHere’s medical clinic team in providing counsel, recommendations and reporting
metrics for public and private employer-sponsored medical clinics. Mr. Tomlinson
brings research and developmental experience from the management consulting, healthcare
insurance, human resource settings.
Mr. Tomlinson holds undergraduate and graduate degrees from Rollins College. He
has more than twenty-four years experience in the areas of management, healthcare
insurance, human resource fields and nearly twelve years experience as an investigator
for governmental agencies.
Laura Weaver, MBA
Director of Client Operations
Mrs. Weaver joins CareHere after working over a decade with the Tennessee Department
of Health. She has an expansive background in public health service and wellness
initiatives. Mrs. Weaver counseled patients, ranging in age from infant to adult,
on the importance of proper nutrition and wellness.
Most recently, Mrs. Weaver’s primary role was as the Public Health Educator for
several surrounding counties. During this time, she was instrumental in developing
and implementing health and wellness presentations, policies and curriculums for
various organizations such as schools, churches, civic and community groups.
Mrs. Weaver graduated from the University of North Alabama with a B.S. in Food and
Nutrition and Business Management. She received her MBA from Union University in
Jackson, TN.
Rhonda Wood, RN
Health Coach
Mrs. Wood is a registered nurse and health coach for CareHere patients. She has
27 years experience in nursing ranging from a variety of acute care settings in
Medical, Surgical, Neurology, Urology, Orthopedic nursing, Spinal Cord Injury Rehabilitation,
and Pain Management. Her most recent experience prior to becoming a health coach
was with the YMCA of Middle Tennessee and St. Thomas Health Systems alliance. In
this role she developed health programming for all age groups and educated patients
on the importance of preventive health measures.
Ms. Wood holds a nursing degree from Texas Eastern School of Nursing in Tyler, TX.
