Denise Agee, RD, LDN
Registered Dietitian, Director of Health and Wellness
Mrs. Agee is a registered dietitian and health coach for CareHere patients. She
has expertise in areas such as weight management, cardiac rehab, bariatric nutrition,
and diabetes nutrition counseling. Prior to working for CareHere, Mrs. Agee worked
as an outpatient dietitian at a local hospital. As part of the CareHere team, she
leads group classes on nutrition related topics and also meets one on one with patients
to develop individualized nutrition plans. Mrs. Agee has also worked to develop
protocols for diabetes education, weight management and cholesterol management.
Mrs. Agee holds a B.S. in Family and Consumer Sciences with a dietetics concentration
from Lipscomb University. She also completed the dietetic internship program from
Lipscomb University. She is a licensed dietitian/nutritionist in Tennessee, Ohio,
South Carolina and Mississippi. She is a member of both the American Dietetic Association
and Tennessee Dietetic Association.
Lee Anglea
Vice President, Purchasing
Mr. Anglea has worked in the healthcare purchasing arena since 1989. He has hospital, pharmacy and healthcare commodities experience. He manages the procurement of all supplies for CareHere clinics nationwide as well as contract negotiations and vendor selections.
Michelle Anglea, RNC, OHN, CMC, ICEMA
Senior Vice President & Chief Clinical Officer
Mrs. Anglea has over 16 years of experience in managing and implementing onsite
healthcare operations including primary care clinics, occupational clinics, physical
therapy, and pharmacy operations . Most recently, she worked with several Fortune
50 Companies overseeing medical sites in several states.
Mrs. Anglea holds a Bachelor of Science degree in Nursing, with a minor in Psychology,
from Belmont University. She is an Occupational Nurse Specialist, Certified Case
Manager and holds other various specialty certificates and licensures. Michelle
manages our corporate wide quality improvement and assurance, audit, and annual
training programs.
Mrs. Anglea has deep experience with HIPAA, OSHA, CDC guidelines and NRC regulations
and holds memberships in: American Association of Occupational Health Nurses, American
Health Information Management Association Member, Associate Member of MGMA, American
Academy of Professional Coders, CMSA (Case Management Society of America) Member,
Tennessee Association of Occupational Health Nurses, and Red Cross CPR instructor.
Ben Baker, MBA
Chief Operating Officer
Mr. Baker brings 18 years of experience in customer service, information technology,
quality management, and business process improvement from General Electric and IBM
to CareHere.
Mr. Baker is the former President of Precision Data Management Systems providing
Information Technology consulting services. He is a certified Master Blackbelt in
Six Sigma Quality pioneered by GE.
Mr. Baker is a graduate of David Lipscomb University and received his MBA from the
Owen Graduate School of Management of Vanderbilt University.
Mr. Baker and Ernie Clevenger co-founded CareHere in January 2004.
John Boyd
Director of IT
Mr. Boyd has been a software developer for over 8 years and his experience has spanned
over several industries including manufacturing, footwear, and healthcare. His software
development has included web, client-server, and database applications, as well
as database administration. Several of his past projects have been conversions of
legacy applications to newer technologies, such as AS/400 applications to ASP.NET
applications.
Mr. Boyd graduated from Tennessee Technological University in 1998 with a Bachelors
of Science in Business Administration-Management Information Systems.
Janice Clary
Purchasing Coordinator
Mrs. Clary is a purchasing agent for CareHere. Her specialty is customer service. She graduated from David Lipscomb University with a Bachelor’s Degree in Health and Physical Education. Her previous work experience includes Speight and Parker Law Firm, The Metropolitan Davidson County School System and The Metropolitan Government with the most recent being the Davidson County Clerk’s Office where she trained new employees and wrote their training procedures manual.
Ernie Clevenger, MBA, FLMI
President
Since 1983, Mr. Clevenger has worked in the self-funded community in various capacities
including:
- Chairman and President of the Self-Insurance Institute of America.
- Chairman of the Workforce for Electronic Data Interface (WEDI) advising United States
Department of Health & Human Services.
- President, American Progressive Benefits and Executive Vice President of Alliance
Underwriters, two of the nation's leading providers of stop loss insurance for self-insured
employers.
Mr. Clevenger is a graduate of David Lipscomb University and received his MBA from
the Owen Graduate School of Management of Vanderbilt University. He received his
FLMI from the Life Management Institute.
Mr. Clevenger and Ben Baker co-founded CareHere in January 2004.
Anthony V. Dallas, Jr, MD
Chief Medical Officer
Dr. Dallas also practices as a CareHere physician providing onsite care for clients
and overseeing nurse practitioners.
Dr. Dallas attended the University of Kentucky Medical School (Graduated with Honors)
and performed his residency at St. Mary’s Medical Center in Evansville, Indiana.
He is the founder of the Family Healthcare of Hendersonville (Hendersonville, TN)
and is Board Certified by the American Board of Family Practice.
Dr. Dallas holds memberships in: the American Academy of Family Practice (1975),
the American Academy of Medicine (1980), the Sumner County Medical Society (1986),
and the Tennessee Medical Association (1986).
Liza Derbalian, RD, LD
Registered Dietitian, Director of Health and Wellness
Ms. Derbalian is a registered dietitian and health coach for CareHere. She has expertise
in areas such as weight management, home care and long-term care nutrition, diabetes
and cardiac nutrition counseling. Prior to joining CareHere, Ms. Derbalian worked
as a clinical dietitian for a Florida hospital and as a consultant dietitian at
a long-term care facility. She has also held several other positions in the past
including: bariatric nutrition and weight management counseling at a major teaching
hospital and outpatient nutrition management at a fitness center in California.
As part of the CareHere team, Ms. Derbalian leads group classes on nutrition related
topics and also meets one on one with patients to develop individualized nutrition
plans.
Ms. Derbalian holds a BS in Nutrition from Rutgers University in NJ, and she completed
her dietetic internship at Montclair State University in NJ. Ms. Derbalian is a
Licensed Dietitian in the state of Florida and is also a certified Adolescent Weight
Management Counselor with the American Dietetic Association.
Amanda Gibbs
Operations Supervisor
Mrs. Gibbs joined CareHere in November 2010 to support the directors of client operations nationwide as the Operations Supervisor.
Additionally, she serves as assistant to the Sr. Vice President and Chief Clinical Officer. Among her many duties, Mrs. Gibbs is responsible for policy and procedure updates,
drug and alcohol testing accounts, patient satisfaction surveys and CLIA certification.
After graduating from the University of Tennessee, Knoxville with a Bachelor’s Degree in Private and Commercial Recreation in 1995, Mrs. Gibbs returned to her hometown
of Nashville, TN. Using outstanding project management, marketing, organizational and customer service skills, Mrs. Gibbs most recently served the YMCA of Middle TN and Agee, Van Atta & Owens Law Firm.
JeNelle Gouvas
Vice President of Recruitment & Provider Relations
Mrs. Gouvas comes to CareHere from a major pharmaceutical company where she served
as Executive Sales Representative. She was instrumental in the development of training
protocols for new sales representatives, sales, marketing and physician relations.
Mrs. Gouvas will serve in a variety of roles with CareHere, with emphasis on sales,
marketing, physician relations and account management.
Born and raised in Owensboro, Kentucky, Mrs. Gouvas graduated from Western Kentucky
University with a degree in Corporate and Organizational Communication in 1994.
Bradford Hancock
Director of Recruitment
Mr. Hancock joined CareHere in February of 2011 as a new part of the recruitment team, focusing on healthcare providers. Prior to CareHere, Bradford was a
professional sales representative in the pharmaceutical market space.
Mr. Hancock is originally from Hopkinsville, KY and is a graduate of the University of Kentucky. He resides in
Franklin, TN with his wife and two children. Bradford is a part owner of a nationally recognized blog and his interests include reading, athletics, serving and being a husband and father.
Lauren Heath, RD, LDN
Registered Dietitian, Director of Health and Wellness
Ms. Heath is a registered dietitian and health coach for CareHere patients. She
has expertise in the areas of weight management, diabetes nutrition counseling,
and dietary modification for a variety of medical diagnoses. Prior to working for
CareHere, Ms. Heath worked with patients at a local outpatient diabetes education
program. As part of the CareHere team, she leads educational group classes on nutrition
related topics and also meets one on one with patients to develop individualized
nutrition plans. Ms. Heath has also worked to develop and implement protocols for
diabetes education, weight management, and cholesterol management.
Ms. Heath holds a B.S. in Nutrition with a minor in Business Administration from
the University of Tennessee at Knoxville. She completed her dietetic internship
at Lipscomb University. Ms. Heath is a certified Adolescent Weight Management Counselor
with the American Dietetic Association, and a Certified Wellness Coach with Wellcoaches
Corporation. She is a member of both the Nashville District Dietetic Association
as well as the American Dietetic Association.
Sarah Houser, CPA
Chief Financial Officer
Mrs. Houser is responsible for all financial reporting for CareHere. Additionally,
she handles client billing and payroll. Mrs. Houser joined CareHere in 2004 after
providing accounting services to area start-up companies for five years. Prior to
that she was a senior auditor with Ernst & Young, where she worked primarily with
healthcare companies.
Mrs. Houser is a Certified Public Accountant in the state of Tennessee. She holds
a Bachelors of Science in Business Administration from Auburn University.
Megan Jakes
Recruitment Specialist
Ms. Jakes graduated from the University of West Florida in May 2007 with a Bachelors of Arts and Science specializing in Public Relations and a minor in Hospitality, Recreation and Resort Management. Upon graduation, she moved back to her hometown area of Nashville and entered into the corporate world as a Newspaper Liaison with Webster & Associates, an entertainment Public Relations firm. She then moved to HMG Nashville, a radio promotion company. Starting out, Ms. Jakes handled radio promotions for all artists, as well as press releases on specific artists. She was then promoted to Artist Representative in the Sales department and dealt directly with artists and created a development plan for success. Ms. Jakes made the move to CareHere in February 2009 and brings with her organizational, public relations, crisis management and marketing skills.
Brendie Keane, RN
Vice President, Health and Wellness
Ms. Keane oversees daily operation of the disease and case management for the patients
of CareHere. She also develops relationships with local health and wellness facilities
to support telephonic disease management of CareHere patients in their own community.
Her most recent work was leading a multi-site health initiative with the YMCA of
Middle Tennessee and Saint Thomas Health Services. She has over a decade of experience
in managing Vanderbilt Allergy and Asthma Research Center where she lead clinical
research initiatives and developed disease management programs for a variety of
respiratory disorders.
Ms. Keane holds a B.S. in Nursing from East Tennessee State University.
Phil Lander
Technology Coordinator
Mr. Lander brings over 15 years experience in Information Technology to CareHere.
His experience has spanned several industries including legal, manufacturing, and
the last 9 years in healthcare. He has supported telecommunication systems, client-server
networks, end user systems, as well as managing support teams. In his current role,
Mr. Lander supports the daily infrastructure needs of CareHere, as well as participating
in strategic planning for its growth.
Mr. Lander graduated from the University of Montevallo with a Bachelor’s degree
in Speech-Language Pathology, as well as a Master’s in Audiology.
Jason Pack
Purchasing Coordinator
Mr. Pack brings deep experience purchasing for occupational, primary care, pharmacy,
and corporate health facilities. Most recently, he worked for a organization that
managed hundreds of clinics in 35 different states. For CareHere, Mr. Pack's responsibilities
include: processing supply orders, product research, and order tracking for all
of the CareHere clinics. Furthermore, he oversees formulary pricing for the Director
of Pharmacy Operations.
Mr. Pack holds a Bachelors of Science degree in Exercise Science and Wellness from
Jacksonville State University.
Kimberly Rieger
Executive Assistant
Mrs. Kimberly Rieger brings eight years of office management experience to CareHere. Kim graduated from Trevecca Nazarene University with a Bachelor of Science in Music Business. She started her career working for Tom Collins Productions, where she filled the roles of Publishing Administrator, Creative Manager, and Executive Assistant to Mr. Tom Collins. While at TCP, Kim was responsible for managing the current catalog, processing incoming royalty statements, and handling licensing requests, as well as day-to-day operations of the office and public relations events. Kim joined the CareHere team in April 2011 and brings with her extensive experience in office management and customer service. In her role at CareHere she will handle administrative duties for all staff, as well as directly assisting the President and COO of the company.
Brooks Smith, PharmD
Vice President of Pharmacy Services
Dr. Smith brings 12 years of pharmacy experience to his role as pharmacy operations manager. Recently he has been a director of pharmacy services for a hospital pharmacy and has previously been a manager of a retail pharmacy. Dr. Smith will assist on pharmacy issues including formulary management and providing drug information to providers, patients, and clients. Dr. Smith was a cum laude graduate of Mercer University Southern School of Pharmacy in Atlanta, Georgia and is a member of several professional organizations.
Ray Tomlinson
Vice President, Florida Operations
Mr. Tomlinson provides executive oversight and direction for CareHere’s Florida
operations from the initial presentation stage through implementation. He works
with CareHere’s medical clinic team in providing counsel, recommendations and reporting
metrics for public and private employer-sponsored medical clinics. Mr. Tomlinson
brings research and developmental experience from the management consulting, healthcare
insurance, human resource settings.
Mr. Tomlinson holds undergraduate and graduate degrees from Rollins College. He
has more than twenty-four years experience in the areas of management, healthcare
insurance, human resource fields and nearly twelve years experience as an investigator
for governmental agencies.

Betsy D'Acierno
Executive Director, Texas Operations
Ms. D'Acierno has worked for CareHere since February 2007 and currently responsible
for all clinic operations in Texas. She brings more than 17 years combined health
care and medical management experience to serve Texas clients.
As a market development manager for Ethicon Endo-Surgery, a Johnson & Johnson company,
Ms. D'Acierno worked for more than 8 years with Surgeons to advance their surgical
skill set, in both open and laparoscopic surgery. In addition, she served as a consultant
to Surgeons to provide them counsel on marketing initiatives and patient education
to expand their practices.
Prior to her tenure at Johnson & Johnson, Ms. D'Acierno worked as Director of Texas
Communications for United HealthCare, managing member benefits' education and communications.
She is a graduate of the University of Texas at Austin.

Brandie Lee
Director of Client Operations
Mrs. Lee joined CareHere in February 2011 after spending several years as the Director of Operations for Pinnacle Medical Solutions,
a regional durable medical equipment provider that specialized in diabetes testing supplies and insulin infusion pump therapy.
She was involved in building the company from the ground up and was responsible for the daily operations of the company
as well as vendor relations, staff training and development, and inside sales. Prior to her employment at Pinnacle Medical Solutions,
Mrs. Lee held positions as Compliance Manager and Director of Operations at The Diabetes Store, a local diabetes equipment supplier
and diabetes education center.
As a member of the CareHere team, Mrs. Lee is responsible for the operations of all clinics located in Mississippi.
Mrs. Lee is a graduate of Mississippi State University where she earned a Bachelor of Science degree in 2002.
Colleen Johnson
Director of Client Operations
Mrs. Johnson brings management experience in customer relationships, sales and marketing, business development, conflict resolution, accounting, and personnel training and development.
Her background includes knowledge of the healthcare, commercial real estate, restaurant, hospitality, self-storage, billboard, and residential real estate industries.
Most recently, Mrs. Johnson has been an Account Executive with Home Buyers Warranty Corporation, and has been responsible for all sales and client management in Middle TN, East TN,
and Northern AL with a customer base of over 375 offices.
She has served on several boards of directors including the Williamson County Association of Realtors, Junior League of Nashville, and Harpeth Hall School Alumnae Board.
A Nashville native, Mrs. Johnson is a graduate of Vanderbilt University.
Courtney Leech
Director of Clinic Operations
Mrs. Leech joined CareHere is November 2010. She has over 20 years of healthcare management experience both in the private and public sector. Mrs. Leech graduated from the University of South Florida with a BA in Psychology. She received her MBA in Human Resource Management from the University of Phoenix.

Jamie Wagner, RN, BSN
Director of Client Operations
Mrs. Wagner joins CareHere with 15 years of management experience in Ohio and Florida. As a medical assistant, she worked in Orthopedics for 5 years. Later as an RN she worked mostly in the NICU and Pediatrics before becoming the manager of four Community Health Centers in central Florida. She has also completed her BSN from Chamberlain College of Nursing. She received her BSN from Chamberlain College of Nursing.
Laura Weaver, MBA
Senior Director of Client Operations
Mrs. Weaver joins CareHere after working over a decade with the Tennessee Department
of Health. She has an expansive background in public health service and wellness
initiatives. Mrs. Weaver counseled patients, ranging in age from infant to adult,
on the importance of proper nutrition and wellness.
Most recently, Mrs. Weaver’s primary role was as the Public Health Educator for
several surrounding counties. During this time, she was instrumental in developing
and implementing health and wellness presentations, policies and curriculums for
various organizations such as schools, churches, civic and community groups.
Mrs. Weaver graduated from the University of North Alabama with a B.S. in Food and
Nutrition and Business Management. She received her MBA from Union University in
Jackson, TN.

Brandi Aiken, RN
Registered Nurse, Health Coach
Ms. Aiken is a Registered Nurse and Health Coach for CareHere patients. She has 14 years of experience in the health field ranging from long term care management, care for adults
with developmental disabilities, and in neuroscience assisting stroke and brain injured patients. Ms. Aiken has a diverse background including marketing and grant writing as the
Director of Development at Annandale at Suwanee, Inc., where she raised in excess of $2.5 million for a nonprofit organization serving adults with developmental disabilities
allowing the organization to expand programs and services. Most recently, Ms. Aiken has served terminally ill patients and their families as a hospice case manager
providing emotional support and symptom management.
Ms. Aiken received her nursing degree from North Georgia College and State University and holds a Business Degree from Gainesville State College.
She also holds a Certificate of Excellence in Nonprofit Leadership and Management from Michigan State University.
Kristin Bitter, RD, LD, CSR
Registered Dietitian, Health Coach
Kristin Bitter is a Registered Dietitian and Health Coach for CareHere. She holds a specialty certification in renal nutrition. Prior to joining the CareHere team, Kristin worked for Mayo Clinic in Jacksonville, Florida where she served as the dietitian for their outpatient dialysis population as well as the dietitian for the Chronic Kidney Disease Clinic where she counseled patients with chronic kidney disease, diabetes and hypertension. As part of the CareHere Wellness Team, Kristin meets individually with clients and leads group classes on a variety of nutrition related topics. Kristin holds two Bachelor’s Degrees in Health Science from the University of North Florida – one with a concentration in Public Health and the other with a concentration in Nutrition and Dietetics. She completed her dietetic internship at Mayo Clinic in Jacksonville, Florida. Kristin is a licensed dietitian in the state of Florida and is a member of the American Dietetic Association, Palm Beach Dietetic Association, and Renal Practice Group of the American Dietetic Association.

Jessica Dow is a Registered Dietitian and Health Coach for CareHere. She has an expertise in areas such as weight management, diabetes and cardiac nutrition counseling, long-term care nutrition and pediatric nutrition. Prior to working for CareHere, Ms. Dow was a consultant dietitian in a variety of health care settings including long-term care, behavioral health facilities and pediatric outpatient clinics. As part of the CareHere wellness team, Ms. Dow counsels clients individually as well as conducts groups and presentations on a variety of nutrition-related topics. Jessica holds a Master’s degree in Health Science with a concentration in Nutrition and Dietetics from the University of North Florida. She completed her dietetic internship in Jacksonville, FL at the University of North Florida as well. Ms. Dow is licensed Dietitian in the state of Florida and is a member of the Orlando area Dietetic Association, American Dietetic Association and Weight Management Dietetic Practice Group.
Erik Frankwich, LCSW
Health Coach
Erik Frankwich, LCSW, is a licensed clinical social worker and health coach for CareHere patients.
Previous to working at CareHere, he worked in a variety of settings to deliver social work services, including,
counseling, psychotherapy, life coaching, and case management. Before becoming a licensed clinical social worker,
Erik was a certified acupressure practitioner where he worked with patients in chronic pain and with mental health issues.
Erik started his career in social work by treating the chronically mentally ill and their families for both mental health issues and
substance abuse. More recently, Erik has specialized in issues relating to children, teens, adults, couples, and families as a psychotherapist.
He has treated issues such as substance abuse, depression, and anxiety using Cognitive-Behavioral Therapy, Solution Focused Therapy,
Crisis Intervention, and Narrative Therapy.
Erik received his Master’s of Social Work from the University of Central Florida. He continues to expand his knowledge of
social work and psychotherapeutic techniques. Currently, he has been utilizing Motivational Interviewing Techniques to assist
clients in finding their internal motivation to make positive, long-term changes in their lives.
Jinny Glasco
Social Worker
Jinny Glasco is a Licensed Clinical Social Worker in the state of Tennessee. She is a behavioral health and tobacco
cessation coach for CareHere patients. She comes with experience in the educational and private practice setting having
expertise in many areas of the mental health field, including work with children and adolescents dealing with severe mental
health and addiction issues. She also has experience working with adults who have anxiety and depression. Most recently, her
experience has been in an educational setting as a Positive Behavioral Support Coach for teachers and staff working with special education students.
As part of the CareHere Wellness team, she is working with patients to educate them for better management of stress, anxiety and depression.
She works with the health coaching team and patients to identify community resources for meeting emotional health needs. In addition, she facilitates
groups for stress reduction, tobacco cessation and emotional eating issues.
Mrs. Glasco holds a Bachelors of Social Work from Freed Hardeman University in Jackson, Tennessee. She received her Masters of Social
Work from The University of Tennessee, Nashville Campus and was a member of the Phi Alpha Honors Society. She received her LCSW in 2006
and has been very involved in her community serving as a member of a Foster Care Review Board and Child Abuse Review Team. She has a passion
for advocacy and helping people make changes in their lives.
Missy Hagy
Care Management Coordinator
Mrs. Hagy is a care management coordinator providing assistance to patients who
are struggling to begin the steps to making necessary health changes. She will guide
patients in the process of choosing services to help move them along the path of
change while preparing them to work with the appropriate health coach. She will
assist patients from across the country to register for programs best suited for
their needs. She will assist in coordination of special programs such as flu shots,
webinars, and health risk assessment events. Mrs. Hagy will also develop patient
friendly marketing materials for CareHere programs.
Mrs. Hagy has been working in the health and fitness industry for the past 20 years.
She has worked as a personal trainer, group exercise instructor, wellness director
for various health and wellness facilities. Mrs. Hagy has developed and implemented
exercise programs for various clientele and special populations. Mrs. Hagy holds
a B.A. in Broadcast Journalism from the University of Arkansas and is a certified
personal trainer through American Fitness Training of Athletics (AFTA).
Lacy Haney, MS, RD, LD
Registered Dietitian, Health Coach
Lacy has her Bachelors of Science in Nutritional Sciences from Texas A&M University. She completed her Dietetic Internship at Texas Tech University and got her Masters of Science in Nutrition at Texas Women’s University.
Sarah Hankes, RD, LD, CDE
Registered Dietitian, Health Coach
Sarah Hankes is a Registered Dietitian and Certified Diabetes Educator for CareHere.
She has expertise in weight management, cardiac care, eating disorders, and all
areas of diabetes self management. Before joining CareHere, Ms. Hankes worked as
the Education Coordinator with Diabetes and Endocrinology Associates in Decatur,
GA. She has held many positions in both inpatient and outpatient settings and in
private practice. As part of the CareHere team, Ms. Hankes meets individually with
clients and leads group classes on a variety of nutrition related topics.
Ms. Hankes holds a BS in Food and Nutrition from the University of Alabama, Tuscaloosa,
AL. She is currently a licensed dietitian in Alabama and Georgia.
Lauren Heath, RD, LDN
Registered Dietitian, Director of Health and Wellness
Ms. Heath is a registered dietitian and health coach for CareHere patients. She
has expertise in the areas of weight management, diabetes nutrition counseling,
and dietary modification for a variety of medical diagnoses. Prior to working for
CareHere, Ms. Heath worked with patients at a local outpatient diabetes education
program. As part of the CareHere team, she leads educational group classes on nutrition
related topics and also meets one on one with patients to develop individualized
nutrition plans. Ms. Heath has also worked to develop and implement protocols for
diabetes education, weight management, and cholesterol management.
Ms. Heath holds a B.S. in Nutrition with a minor in Business Administration from
the University of Tennessee at Knoxville. She completed her dietetic internship
at Lipscomb University. Ms. Heath is a certified Adolescent Weight Management Counselor
with the American Dietetic Association, and a Certified Wellness Coach with Wellcoaches
Corporation. She is a member of both the Nashville District Dietetic Association
as well as the American Dietetic Association.
Brendie Keane, RN
Vice President of Health and Wellness
Ms. Keane oversees daily operation of the disease and case management for the patients
of CareHere. She also develops relationships with local health and wellness facilities
to support telephonic disease management of CareHere patients in their own community.
Her most recent work was leading a multi-site health initiative with the YMCA of
Middle Tennessee and Saint Thomas Health Services. She has over a decade of experience
in managing Vanderbilt Allergy and Asthma Research Center where she lead clinical
research initiatives and developed disease management programs for a variety of
respiratory disorders.
Ms. Keane holds a B.S. in Nursing from East Tennessee State University.
Rhonda Livers, MA
Health Coach
Ms. Livers comes to CareHere with over 20 years of experience working in various
settings in the field of addictions. As part of the CareHere team, she is a health
coach for the tobacco cessation program. Rhonda leads group workshops on tobacco
cessation, trains and oversees other health coaches and facilitators who also implement
the CareHere tobacco cessation program nationally. Her most recent achievement was
developing a tobacco cessation program for CareHere.
Ms. Livers currently holds a MA in Christian Counseling Psychology from Carolina
University of Theology and a MA in counseling from Trevecca Nazarene University
in 2009.
Susie Loomis
Health Coach
Susan is a Registered Nurse and Health Coach for CareHere patients. She has twenty years experience in nursing ranging from Medical, Surgical, Pediatric, Geriatric, and Case Management. She holds a nursing degree from Grays Harbor College in Aberdeen, Washington and is currently attending South University to further her education.
Emily Maddux, MS, MPH, RD, LDN
Registered Dietitian, Health Coach
Mrs. Maddux is a Registered Dietitian and Health Coach for CareHere patients. She
has expertise in Medical Nutrition Therapy, specifically with nutrition support,
gastrointestinal disease nutrition management, and diabetes and cardiac nutrition
counseling. Mrs. Maddux worked as a Clinical Dietitian in an acute care hospital
in Memphis, TN, and then moved to Chattanooga, TN where she worked at a local hospital.
As part of the CareHere team, she meets one on one with patients to develop individualized
nutrition plans to help with weight management, diabetes management, lipid management
and other diseases/conditions. Mrs. Maddux also leads group classes on various nutrition
topics.
Mrs. Maddux received a Bachelor of Science degree from Lipscomb University. She then went
to the University of Tennessee-Knoxville, where she completed the Dietetic Internship
Program and received a Master in Science degree and a Master of Public Health degree.
Mrs. Maddux is a Licensed Dietitian/Nutritionist in the state of Tennessee. She
is a member of the American Dietetic Association and Tennessee Dietetic Association,
and she is currently the President of the Chattanooga District Dietetic Association.
Julie Mason, RD

Julie Mason is a registered dietitian and health coach for CareHere. Prior to working for CareHere, Julie worked as a
clinical dietitian for 5 years at University Medical Center in Lebanon, TN. As part of the CareHere team, she leads group
classes on a variety of nutrition topics and meets one on one with patients to address individual nutritional needs.
Julie holds a B.S. degree in Family and Consumer Science with a concentration in Dietetics and a minor in Distributive Science
from Lipscomb University. She completed the Dietetic Internship Program at Lipscomb University. Julie is licensed in Tennessee, Kentucky, Pennsylvania, and Maryland.
Ryann Matthews, RD
Registered Dietitian, Health Coach
Ms. Matthews is a Registered Dietitian and Health Coach for CareHere patients.
She has expertise in areas of weight management, diabetes management, and sports nutrition.
Prior to working for CareHere, she provided nutrition counseling at a wellness center that
specialized in medically supervised weight loss. As part of the CareHere team,
Ms. Matthews meets with patients one-on-one to develop individualized nutrition plans for
improved health and wellness. She also leads group classes on various nutrition related topics.
Ms. Matthews graduated from Florida State University where she received her Bachelor of
Science degree in Dietetics. She received her Master of Science degree in Clinical Nutrition
with an emphasis in Sports Nutrition and completed her Dietetic Internship there as well.
She enjoys being active, nutritious food, educating and helping others make positive lifestyle changes.
Kathy Mead, MS
Exercise Physiologist, Health Coach
Ms. Mead is an exercise physiologist and health coach for CareHere patients. She
comes with experience in an acute care wellness center facility, having assisted
patients as they carry out exercise programs outlined by their provider. She works
individually with patients helping to begin or to expand their exercise program
as they work toward weight management goals, pre-natal and post-natal fitness, or
to prepare for bariatric surgery. Additionally as part of the CareHere team, she
serves as program coordinator for special events for clients, with particular expertise
in Health Risk Assessment and flu shot clinic events.
Ms. Mead holds a Bachelors of Science degree in Psychology and Human and Organizational
Development from Vanderbilt University and a Masters of Science in Exercise Science
from Middle Tennessee State University. She is a member of the American College
of Sports Medicine.
Uthona Shelton, MS
Wellness Coordinator
Uthona Shelton joined CareHere as the Florida-based Wellness Coordinator in September
2010. Ms. Shelton received her B.S. in Nutrition from Florida State University in
2002 and M.S. in Nutrition from Texas Woman’s University in 2004. Throughout graduate
school she worked as a graduate teaching assistant, teaching undergraduate courses
and assisting with departmental research.
Following graduation, Ms. Shelton gained experience working in a variety of corporate
and small business settings, including working with Xerox as a marketing representative,
directing marketing and client coordination as a licensed real estate team member
with the Nashville based Zeitlin & Co., Realtors, and directing sales and marketing
for mimijumji, LLC., a baby product design company. Her other professional experiences
include personal training and weight loss counseling.
The Wellness Coordinator position is one that will encompass elements Ms. Shelton
finds most enjoyable and challenging on a daily basis, including detail management,
planning, teamwork, and teaching.
Tracy Thompson, RN
Registered Nurse, Wellness Initiative Coordinator
Mrs. Thompson holds nursing licenses in Tennessee, Florida and received a Bachelor of Science in Nursing from the University of South Carolina in Spartanburg, South Carolina and Tennessee.
Cynthia Vallo, M.Ed., RD, LD
Registered Dietitian, Health Coach
Ms. Vallo is a registered dietitian and health coach for CareHere patients. She has
expertise in the areas of weight management, eating disorders, heart disease nutrition
counseling, and dietary modification for a variety of medical diagnoses. Prior to working
for CareHere, Ms. Vallo was Program Director for the American Lung Association of Southeast
Florida for 12 years. She has also held several other positions in the past including:
Program Associate for the American Lung Association, Ohio Affiliate; outpatient treatment
for eating disorders at Good Samaritan Medical Center in West Palm Beach, FL; and prenatal
and pediatric nutrition counseling. As part of the CareHere team, she leads educational
group classes on nutrition related topics and also meets one on one with patients to develop
individualized nutrition plans.
Ms. Vallo holds a M.Ed. and B.S. in Nutrition from the University of Cincinnati. She completed
her dietetic internship at Cincinnati Center for Developmental Disorders, Cincinnati Children’s
Hospital Medical Center. Ms. Vallo is a Licensed Dietitian in the state of Florida and a member
of the American Dietetic Association
Rhonda Wood, RN
Registered Nurse, Health Coach, Team Leader
Mrs. Wood is a registered nurse and health coach for CareHere patients. She has
29 years experience in nursing ranging from a variety of acute care settings in
medical, surgical, neurology, urology, orthopedic nursing, spinal cord injury rehabilitation,
and pain management. While with CareHere, she has helped to develop evidenced based
protocols to assist CareHere patients as they meet health goals including diabetes
management, cholesterol reduction, and support of the depressed patient. She has
assisted in developing programs to assist families caring for the aging parent.
She has also been instrumental in creating a library of online videos to educate
patients on health care needs.
Ms. Wood holds a nursing degree from Texas Eastern School of Nursing in Tyler, TX,
and is a Certified Wellness Coach with Wellcoaches Corporation.

Bernie comes to CareHere with 25 years of sales management/sales force development background. His expertise will be utilized to develop the CareHere expansion of sales and marketing. Since graduating from the University of Kentucky, Mr. Livers has developed multiple sales organizations.
Chris Murphy

Mr. Murphy is a graduate of Lipscomb University with a Bachelors of Science concentrating in Business Management and Communications.Mr. Murphy brings experience to CareHere in managing on-site clinics in the correctional healthcare industry. He played numerous roles including: opening new clinics, new team member orientation, customer service to field members, and management of operational reports. Mr. Murphy's role will be to focus on new business development.
